JOB TITLE:     License Vocational Nurse                                          REPORTS TO:    RN Supervisor

DEPARTMENT: Home Health Agency

FUNDAMENTAL PURPOSE OF THE JOB:

The Licensed Vocational Nurse (LVN) administers skilled nursing care to patients requiring intermittent nursing service performed in accordance with the physician's orders and under the supervision of the Registered Nurse (RN).

ESSENTIAL FUNCTIONS:

   1. Assists in developing and implementing the plan of care for the patient.
   2. Provides services in accordance with agency policies and procedures.
   3. Assist physicians and I or RN in performing specialized procedures.
   4 Maintains patient charts and records. Completes nursing notes daily and writes
      supplemental orders, if necessary, and assists with case conferences.
   5 Reports daily to RN supervisor, regarding progress toward patient's anticipated    outcomes.
   6. Improves patient care through participation in ongoing education and Quality Assurance
      activities as assigned.

OTHER JOB FUNCTIONS:

Services on Agency Committee as requested.

JOB REQUIREMENTS:

   

A. NONPHYSICAL

      1. Education
          Graduate of an accredited school of Vocational Nursing.

      2. Experience
          Minimum of two (2) years of clinical experience as Licensed Vocational Nurse in an acute care setting.

      3. Special Skills
          Good written and verbal communications skills

      4. Licensure, Registration, Certification
          Current LVN License in state of Texas

      

Current state driver's license and reliable transportation
Automobile insurance coverage
Current CPR certification

ESSENTIAL ENVIRONMENTAL CONDITIONS:

Work involves continuous exposure to unpleasant working conditions or undesirable elements that may involve exposure to a verity of components, that are potentially hazardous or harmful, that require exercise of well-defined precautions for protection from injury or illness, and may occasionally involve exposure to extreme driving and weather conditions.

ESSENTIAL JOB KNOWLEDGE:

Work requires oral grammar and basic communications skills in spelling, grammar and punctuation for completing basic records and reports; basic knowledge of medical terminology; basic arithmetic skills, including multiplication, division, and percentages; and basic understanding of one or more scientific disciplines, such as anatomy, biology, chemistry, physics, and basic disease processes.
